- Who We Help
Care transitions happen at any age
Much of our work is with aging parents, but not all of it. A surgery, an accident, an illness or an addiction can turn a family's life upside down at 30 as easily as at 80. The questions are the same: where will they recover, who will help, and how will it be paid for. We help families answer them.

- 1Get safe first. In an emergency, call 911. For a mental health or substance use crisis, call or text 988.
- 2Understand the level of care. Home with visits, short-term rehab, an inpatient rehab hospital, assisted living, a nursing home or treatment. Each fits a different need.
- 3Find out who pays. Medicare, Medicaid, private insurance, workers' comp and county programs each have their own rules, and most need approval first.
- 4Choose the provider and plan the move home. You usually get to choose. Compare a few options, ask hard questions, and line up equipment and help before the transition.
