Our Story
Family Transition Link was born from a deeply personal experience. When our founder's parents needed care, she found herself overwhelmed by fragmented information, conflicting advice, and the pressure to make critical decisions without adequate support.
That experience revealed a gap in Ohio's elder care landscape: families needed a single, trusted resource—a central hub—that could guide them through every type of transition: housing decisions, home downsizing, care coordination, and financial planning. All with local expertise and a deep understanding of what families are actually going through.
Today, Family Transition Link serves families across seven Southwest Ohio counties as Ohio's transition specialists. We've mapped every step of the journey, so you don't have to figure it out alone. Whether you're planning ahead or facing a crisis, we're here to guide your transition.
