Claim your listing or get vetted. Both are free.

Families in Southwest Ohio use this directory to choose assisted living, nursing homes and elder care professionals. Here's how a business moves from Listed to Vetted.

    Three levels

    Listed

    Free, automatic

How: We add businesses from public sources such as state licensing records and federal CMS data.

What families see: Name, address, phone and public ratings. Families see that the business hasn't been checked by us.

    Verified

    Free

How: Claim your listing. We call your public business number to confirm you work there.

What families see: A Verified badge, and you can correct your details. Verified doesn't mean vetted.

    Vetted

    Free

How: Complete the questionnaire, share three references, and host a short in-person visit.

What families see: A Vetted badge, first placement in results, a summary of what we found, and your phone and website shown prominently.

    Claiming your listing

    1. Find your business in the professional directory, assisted living search or nursing home directory.
    2. On your page, use the Claim this listing form. It asks for your name, role, work email and phone.
    3. We call your business's public phone number and ask for you by name, usually within 2 business days.
    4. Once that's confirmed, your page shows as Verified and you can send us corrections.

    Getting vetted

Four parts, scored out of 100. You need 70 to pass, and the in-person visit is required.

    1. 1

Questionnaire · 20% of the score

About 20 minutes online. For each topic we ask what goes well and what goes wrong, because honest answers about problems tell families more than a list of strengths.

    2. 2

      Public record checks · 30%

We check licenses, complaints, insurance, online reviews and, for nursing homes, federal inspection results and penalties.

    3. 3

      References · 20%

You give us three. We call them ourselves.

    4. 4

      In-person visit · 30%, required

About an hour, scheduled at a time that suits you. We look at the spaces families would see and talk with whoever you choose.

    Ready to start?

You can save the questionnaire and come back. After you submit it, we'll call to schedule your visit.

    Questions businesses ask

    Why is vetting free?
Families trust a badge only if businesses can't pay for it. Charging for vetting would undermine that.
    What if we don't pass?
The pass mark is 70 out of 100. If you score below it, your listing stays as it was and we tell you what held the score back. You can reapply after six months.
    What does 'Excellent' mean?
Vetted businesses scoring 85 or more show "Excellent" next to their badge.
Can we change our CMS star rating or inspection results?
No. For nursing homes, federal data comes straight from CMS and updates monthly. If something looks wrong, CMS and the Ohio Department of Health handle corrections.
    Is paid membership required?
No. Membership adds optional extras such as featured placement, analytics and newsletter mentions. It never affects whether you're Listed, Verified or Vetted, or your vetting score.
    How long does it take?
Claims are usually confirmed within 2 business days. Vetting usually takes 2–4 weeks from questionnaire to decision, depending mostly on references and the visit date.
